Effective 9/13: Passport applications will only be accepted by appointment Wednesdays, 9 a.m. - noon, and Thursdays, 1 - 4 p.m.  Appointments can be made by calling 206.973.4800 a maximum of two weeks prior.

8/24 - 9/7: PASSPORT ACCEPTANCE TIMES -  Passport applications are being accepted Wednesday and Thursday only 9 a.m. to 12 Noon and 1:00 p.m. to 4 p.m. 

The City of SeaTac became a Passport Acceptance Facility in April of 1999, allowing us to assist with applications for a US passport.

By US regulation, only Federal, State or Local government officials may serve as passport acceptance agents. Acceptance agent s receive applications from the public and review them to ensure all necessary information and citizenship evidence is included. They administer the required oath and witness the applicants' signatures, then forward the applications to the Passport Services Agency (US Dept. of State) for issuance. Acceptance facilities also provide the public with general information on passport procedures and serve as a liaison between applicants and Passport Services Agencies. The actual decision to issue or deny a passport is made only by the Federal Passport Services Agency. What Do You Need To Apply?

  • Two identical photographs (2"x2")
  • Proof of citizenship:
    • Certified copy of birth certificate issued by State, County or City; or
    • Previously-issued passport (If you have a previous passport you must bring this with you); or
    • Certificate of Naturalization. - No Photocopies
  • Photo identification such as:
    • Driver's license (no temporaries or learner's permits);
    • Government employee I.D. card;
    • Official Military I.D. card;
    • Aircraft pilot's license issued by DOT, FAA.

We cannot accept social security or medicare cards, credits cards, voter registration, or club membership cards.

New Passport Card

Customers will now have the option to apply for a Passport Book, A Passport Card, or both on the DS-11, DS-82 and DS-5504 forms. The Passport Card may only be used for land and sea travel between the United stated and Canada, Mexico, Bermuda, and the Caribbean.

New Requirements for Children Under Age 16:
Both Parents’ Consent is required for children under age 16 and all minors must appear in person when applying for a passport.

Passport Fees

Passport fees in the form of check/money order (no cash) made payable to U.S. Department of State.
Administrative fees in the form of check/money order/cash (money order preferred) made payable to the City of SeaTac.

*You may use this form if you meet the following criteria:

  1. You can submit your most recent passport
  2. Your previous passport was issued on or after your 16th birthday
  3. Your most recent passport was issued less than 15 years ago
  4. You use the same name as on your most recent passport, or  have had name changed by marriage or court order and can submit proper documentation to reflect name changes
  5. You should use Form DS-11 if your passport is more than 5 years expired from today date, therefore you can not use Renewal Form DS-82
  6. Renewal applications are processed solely by the applicant. The applicant mails the application and all supporting documents directly to Passport Services.
